Insurance companies change their policies and networks every year.  We do our best to update the list below but please check directly with your insurance company to verify coverage. We will bill your insurance provider directly if you supply an updated proof of insurance at each visit. Please note that the insurance policy is between you and your insurance provider; we file the claim as a courtesy to you, but the balance is your responsibility if your plan does not pay.
