All patients receive a copy of the office policies at their first visit, and initial it in agreement to abide by the policies. Dr. Wyatt offers personalized service to all her patients. To help make this possible, please help us help you by honoring the policies below:
MEDICATION REFILLS: Please request these at the time of your office visit. If you need to stop a medication due to side effects, call me to arrange a substitute to be ePrescribed to your pharmacy.
Routine prescription refills are done during office visits only, so please plan ahead and schedule well before you run out. You will be given up to 12 months of refills based on when you are due for follow up. You won't ever need to call your pharmacy for refill requests, and fax requests from pharmacies will not be filled. Failure to comply may results in dismissal from the practice.
PHONE CALLS: Office hours are listed at the left side of this page. If I am with a patient, you may leave a message with one of the staff. I return all calls during business hours before the end of each day. A doctor is available on-call 24 hours a day.
Personal contact is important for good medical care. For this reason, I do not give medical advice by email or phone. Lab results will always be called to you, and are available on the Patient Portal if you'd like to print a copy. Referrals require a visit unless it is regarding something we discussed at your last visit.
MISSED APPOINTMENTS: Please give us the courtesy of 24 hour notice of cancellation. After one missed appointment or cancellation with less than 24 hours notice, there is a $50 fee.
